Pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as the “GDPR Regulation”) and Act No.18/2018 Coll. on the protection of personal data and on the amendment and supplementation of certain acts (hereinafter referred to as the “Act on the protection of personal data”).
By confirming and submitting this document in the “Contact form” section, I grant the operator Advokátska kancelária JUDr. Peter Wolf, s.r.o., seated at Hlavná 949/53, 924 01 Galanta, ID No.: 47 252 987 (hereinafter referred to as the “Controller”) consent to the processing of my personal data for the purpose of free legal advice in the following scope: name and surname, email address (hereinafter referred to as “personal data”). Personal data may be processed on the basis of the consent given. Such processing is permitted under Article 6(1)(a) of the GDPR. For this purpose, I consent to the inclusion in the database of the email addresses of the data controller and their subsequent use for the above purpose.
This consent to the processing of personal data is a freely given, specific,
informed and unambiguous expression of my will, which I give for the necessary period of time,
a maximum of 3 years from the date of sending my enquiry to the free legal advice service, unless
consent is withdrawn earlier.
Before giving my consent, I was informed of the following:
- My personal data, which is provided voluntarily, will be kept for the period of validity of the consent and will not be processed for any purpose other than that for which it was collected.
- As a data subject, I have the right to withdraw my consent at any time, to request information on what personal data is processed about me, to request access to this data and to have it updated or corrected, or to request the restriction of processing or deletion of this personal data, to lodge a complaint with the supervisory authority for the protection of personal data (the Office for Personal Data Protection of the Slovak Republic).
- I understand that there is no cross-border transfer of my personal data to a third country.
- I also acknowledge that there is no automated decision-making, including profiling.
The provision of personal data is voluntary. I

The processing of personal data is carried out by the controller. However, the following entities may also process personal data for
of the controller:
- IT service provider for the operator.
- Provider of bulk emailing software.
- Where applicable, other providers of processing software, services and applications of the controller.
